LVN to RN Nursing

Alert: LVN to RN Step Up 2013 Applicants!

The Nursing Program application period has been reopened until May 3, 2013 at 4:00 p.m.  All applications must be postmarked or hand delivered by the deadline.  Applicants must have all degree and prerequisites completed.  The only exception will be the final LVN semester and licensure. Students accepted without licensure must provide evidence of testing date by June 15, 2013 and licensure by August 2, 2013.

The nursing program will accept applications from those who have been unable to generate a DEGREE WORKS TRANSCRIPT EVALUATION FORM and did submit a transcript 4-8 weeks prior to the application deadline

Applicants who submitted transcripts to enrollment services less than 4 weeks before the deadline may also submit application – specifically LVN applicants after Feb 18, 2013 and RN applicants after March 18, 2013.  The date of transcript receipt by COS will determine how the applicant is ranked in the applicant pool.

Students who secured a transcript evaluation during the fall, 2012 by COS Counseling Services should submit their transcript evaluation  with nursing application to the COS Nursing Program Administration office.   DegreeWorks evaluation is not required.

Applicants must retain copies of all submitted documents.

Applicants must continue to check with the Nursing Program Administration Office on the status of the Degree Works Transcript Evaluation of their transcripts: once available all applicants are responsible for submitting DEGREE WORKS transcript evaluation forms immediately to the Nursing Program Office to complete their application packet.

Associate Degree Nursing (LVN-RN Step-Up) Program
Licensed Vocational Nurses have the opportunity to step up to RN at College of the Siskiyous' state-of-the-art Rural Health Science Institute. The program takes two semesters after completing a 3 unit Transition class during the summer session. Graduates from the program will receive an Associate of Science Degree in Nursing and be eligible to sit for the National Council Licensure Examination-Registered Nursing (NCLEX-RN).
The program admits a new class once a year.

How to Apply

In addition to the college application, a special nursing program application is required. Program applications are available in the column at the left. To learn more about the application process please review the application and program requirement links. If additional information is required, contact the ADN office by calling 530-841-5929. Applicants to the ADN (LVN-RN Step-Up) Program must meet prerequisite and other course requirements before application will be accepted. Prior to submission of a Transcript Evaluation Form students must apply to the college. In order to determine if an applicant meets prerequisites for the program, complete a transcript evaluation request form found here: Transcript EvaluationInstructions: How to Run a DegreeWorks Transcript Evaluation for Nursing Program Prerequisites. Submit official transcripts (unofficial transcripts for COS classes only) from all colleges other than COS to Enrollment Services.  ***Please note that standard evaluation time is 4-8 weeks AFTER official transcripts are received at COS’s Enrollment Services; although evaluations are approached in a timely manner, completion time may be impacted by the prime registration periods. An email explaining how to access DegreeWorks and complete the evaluation will be sent to student when all transcripts have been processed. Include the printout with the Nursing Program application.

 

Applicants will be notified of acceptance during the spring semester prior to the beginning of the summer bridge course.

TEAS Policy

Applicants to the program must complete the TEAS V (Test of Essential Academic Skills) during the application process. The TEAS is offered at no charge to applicants the first time. Students who have completed the TEAS at a college other than COS can request their official results be sent to us for a transcript fee. For more information about the TEAS click here: TEAS Policy.
